I don't think it's necessarily important that you get the exact difference between them, because flavor is mutable, and if you want to re-flavor your druid so that it's drawing upon the elements directly, instead of spirits of nature, that's a perfectly valid thing to do.
And on that note, campaign introduction and sheet!
The Chaos Scar is a long, wide valley carved long, long ago by the fall of a massive meteor. Tales say that as the giant rock passed overhead, milk curdled, livestock fell over dead, and ill fortune befell all. The meteor crashed into the earth with deafening force, and red radiance lit the sky for a week. Then it vanished. The meteor had carved its massive gash in a dark and wild place on the edge of civilization. Where once had stood a wild forest and swamp between a line of small foothills near the western mountains was this new valley—the Chaos Scar.
Many miles long and several hundred feet deep, the Chaos Scar is a testament to the vast power of the meteor’s impact. As it carved its path through the earth, shards and fragments of the meteor tore away until only its heart remained, embedded in an escarpment at the far end of the new valley. Tales say that it was no mere rock, but an alien creature from beyond time and space -- what men call the Far Realm.
The tales must have some truth, for over the centuries, creatures of evil spirit have been drawn to the meteor's heart as if to a beacon. Some say it's dark sentience spurs competition among them so that they fight with one another. The weak are killed or pushed to the edges of the meteor’s influence, while the strong and cruel rise to the top of the pecking order. Vile, ravening hordes have risen again and again, centered around the evil of the Chaos Scar. Hundreds of years ago, one fool in the lands to the east of the Chaos Scar, who called himself a king, thought that he could contain it's evil by building a long wall across the western end of the valley, the ruins of which still bear his title, as the King's Wall. He is long dead, slain on his wall, futilely trying to hold back one of those hordes. It flooded into his kingdom, smashing it to pieces before burning itself out in a frenzy of blood and fire.
Quiet reigned for some time after in the Chaos Scar, though the lords of the land around it were small and petty, bickering with each other, absorbed in their individual quests for power. Now the evil rises again. Hushed tales of a group known as the Brotherhood of the Scar are told in taverns. Restwell Keep, lying near the King's Wall, was until recently the haven of a band of adventurers who were little more than bandits. It was conquered just a few years ago by Lord Peridin Drysdale, a paladin of Erathis, the goddess of civilization and order. Word says that he means to use it as a shield against the evil of the Scar and eventually as a base from which to send forth forces to defeat the evil that dwells there, once and for all. Slightly closer to the Scar lies the town of Bedlam, a haven for the many dubious individuals who make their lives in and around the Scar. Ruled by a lich known as His Honor Mayor Avellin, it has stood since before the creation of the Scar, surviving against the evil that radiates out from the Scar. Many adventurers have entered the Chaos Scar. Few have returned. Those that do weave dark stories of monsters working in concert as never before. Others tell of monstrous creatures working against one another. Welcome to the Chaos Scar. Adventure awaits you there.
